The following are the sequence of prompts to access member’s eligibility verification and claim status quickly:

Dial 1(844) 356-4901

  • Prompt 1 – Press 2 (for providers)
  • Prompt 2 – Press 1 (eligibility /claim status)
  • Prompt 3 – Press 1 (automated system)
  • Prompt 4 – Press 1 (for providers)
  • Prompt 5 – for member’s eligibility verification/benefit – Press 1
    • Be prepared to enter the Member’s ID and Press #
    • If correct, Press 1. If you want a fax copy – Press 2 and enter your fax number (do not enter a leading 1 – i.e. 123-444-5678)
  • Prompt 5 – for claim status – Press 2
    • Be prepared to enter your Tax Identification Number and Press #
    • Be prepared to enter the Member’s ID and Press #
    • Press 1 to receive a Fax or 2 for verbal playback/confirmation (date-of-service will be required)
