Our growing Care Management Team (CMT) is comprised of multiple health care professionals who work hand in hand to meet your healthcare needs.

The Nurse Care Managers provide personalized services to assist you and caregivers in understanding how best to approach your healthcare decisions. We ensure our members have access to needed care, are educated about their healthcare choices and provide resources needed to best manage their health. Our team is dedicated to understanding your unique needs and challenges. We work closely with you to develop a care plan that empowers you to take control of your health. Our Nurse Care Managers provide an individualized member-centered approach to coordinate health care services and will communicate between hospitals, clinics and doctors to best support you, “Our Member”.

We believe that effective care coordination is key to improving health outcomes and is an essential component to improving the quality of life for those living with chronic conditions.
