4 Ways To Be Eligible

It’s important to know whether you are eligible for the Uniformed Services Family Health Plan before you decide to enroll. Here are four ways you may be eligible for the plan:

  • You must be an eligible beneficiary of the Military Health System.
  • You must show as eligible for TRICARE in the Defense Enrollment Eligibility Reporting System (DEERS).
  • You live in New Jersey, parts of Pennsylvania, parts of Connecticut, or parts of New York (New York City, Westchester, Orange, Rockland, Suffolk, Nassau, Putnam, and Dutchess counties).
  • An eligible Active Duty family member or a Uniformed Services retiree under the age of 65. This includes activated members of the National Guard and Reserve and their family members.
