Population Health
What is Population Health?
Population health refers to the health status and outcomes within a group of people, rather than individuals. Population health provides an opportunity for healthcare systems, agencies, and organizations to work together to improve the health outcomes of the communities they serve. The goal of the USFHP Population Health Program is to address our members’ health and well-being including physical, behavioral and social determinants of health (SDOH). We focus on wellness, preventative health, and chronic disease management. We believe a whole-person care approach is essential to enhancing your overall health while addressing the needs of our entire population.

What are the social determinants of health?
- These are non-medical factors that can impact your health and quality of life including:
- Education Access and Quality
- Healthcare Access and Quality
- Neighborhood and Built Environment
- Economic Stability
What is the social determinants of health risk assessment?
- An SDOH risk assessment is a series of questions that help USFHP understand your social needs. USFHP can then provide resources and refer you to the appropriate support services with the help of our Care Management Team

Health Risk Assessments (HRA):
What is a Health Risk Assessment?
- An HRA is a screening tool that gathers information about an individual’s health status, risks, and habits. This helps USFHP identify and understand your health risks and monitor your health status over time. Like an SDOH report, based on the HRA reports, USFHP can then provide resources and refer you to the appropriate support services

- Emotional health
- Physical health
- Preventive screenings
- Current and previous health conditions
How to complete the SDOH and HRA assessment?
- USFHP shares both assessments with all members and asks that you take the time to complete them when joining the Plan. This information is critical in allowing us to better serve you and all your healthcare needs!
